Question:  It appears that there are two bid forms in the package.  Pages 20-24 of pdf (B-1 through B-5) and 25-30 (B-1 through B-6).  Are we to complete & submit both?

Response:  No, only one of the bid forms needs to be completed. The bid form on pages 20-24 of the pdf  (B-1 through B-5) are the pages that should be completed and submitted. Pages 25-30 can be disregarded.

Replace the bid form in the technical specifications with the revised version attached. (The revised version changes the quantity of Item 11 from 13 feet to 67 feet, and changes the quantity of Item 22 from 276 feet to 330 feet.)

Revised Bid Form

In Section 02730 replace Paragraph 1.10 A with the following:

A minimum 8-inch diameter water supply well shall be completed to an estimated total approximate depth of 330 feet, at a minimum the well shall be drilled to a depth sufficient to reach into the limestone formation.

\In Section 02730 Paragraph 3.01 A, change “10-inch diameter hole” to “12-inch diameter hole”.

On Sheet C3 – Well Profile, change the 10” bore hole to a 12” bore hole.

On Sheet C3 – Well Profile, delete the 276’ depth at the bottom of the limestone formation.
