QUESTION: Is the intent to offer the DRE to all diabetic patients, or just those without insurance?  If offering to all, then the target population is 46% of the 4,283 diabetic patients regardless of insurance status?

RESPONSE: The goal is to provide this service to our unfunded patients. We have not ruled out doing it for funded patients should the need arise.

RESPONSE: LCHD hosts it’s Nextgen Application with eMedApps, a certified NextGen hosting company.

QUESTION: Do we fill out and sign the sample agreements?

RESPONSE: No this is strictly a sample agreement for your review of the terms and conditions contained within.

QUESTION: Do we need different proposals for cameras and services or can they all be in the same proposal?

RESPONSE: It can all be included in the same proposal.

QUESTION: Regarding the submission of the proposal, do we upload the proposal to the portal and mail it in, or do we just send a physical copy of the proposal to Lake County Health Department and Community Health Center; Materials Management Department, 2400 Belvidere Road - L50; RFP 21110, Waukegan, IL 60085?

RESPONSE: DO NOT UPLOAD your proposal in the portal, please your hard copy to the location noted in the RFP by the due date and cutoff time.

QUESTION: Do the vendors submit a draft contract with their proposal to replace the SAMPLE document in the packet?

RESPONSE: Vendors may submit a draft contract to include with their proposal. However, this maybe in conjunction with along with the SAMPLE document and included as an Exhibit within the main agreement.
